Referral is easy

Hospice Care at Home

Our services are free of charge to those within our geographical area.  We look after patients who either live in our defined geographical area or who are registered with our partner GP Surgeries.

Referrals are made by the patient’s General Practitioner, District Nurses or the family themselves. Initially, our Nurse Co-ordinator, after liaison with the District Nurse, visits the family to discuss with them the extra nursing help they need. She will then place one of our nurses with the patient.

Once the assessment has been completed by one of our Nursing Co-ordinators, the level of our involvement can be tailored to the needs of the individual.

This creates an opportunity for our nurses to build a relationship with the patient and family at a pace that they are comfortable with.

Cancer & Serious Illness Support Service

This service is also free and available to those living within our geographically defined area (see map below. Referrals can also be made by the GP, District Nurse or family. Alternatively, you can drop in to Jecca’s House where there is always a Registered Nurse on duty to speak to.
